Applicant consent

In order to allow us to process your application using the information that you provide to us, we need your explicit consent to do so.
The information that you provide in the following sections will be used to produce the Baby Loss Certificate(s) that you are applying for, and will then be retained by us (Department of Finance, General Register Office for Northern Ireland) for up to X years, in line with our data retention policies and as described in our Privacy Policy.
